If you would like to become a Volunteer Visitor for the Limbless Association please complete the form below.
To become a Volunteer you must be a full, paying member of the LA with a minimum of one years' experience as an amputee and, having overcome the practical and emotional issues related to your amputation, be in a position to pass on this positive information to others.
During your application you will be asked to provide two references; one from a medical professional, such as your Consultant, Prosthetist, Occupational Therapist, Nurse or GP and the other from a personal or professional contact, such as a colleague, friend or another member of your healthcare team who has known you for a minimum of three years. Such references must contain clear contact details in case we need to get in touch with any of your referees.
Please Note: We are not looking for personal details from your medical history, but for confirmation that you are suitable to provide positive support to other amputees.
You can either scan and upload these references using the online facility below, or post, fax or email them to our VV Supervisor using the contact details found here. Alternatively, your referees can write to us directly.
Your form will be automatically sent to our Volunteer Visitor Supervisor who will process your application and be in touch with you as soon as possible.
